What if I forget to complete my forms?

To make the most of your visit, we ask that forms be completed in advance. If they are not, you may arrive 20 minutes early to complete them in the clinic. If there isn’t enough time to review your information properly, your appointment may need to be rescheduled.

Is my information secure?

Yes. All forms are submitted through a secure, HIPAA-compliant system.

Do I need to complete insurance forms if I’m paying cash?

No. Insurance information is optional for cash-pay patients.
